Fees and Payments

Insurance
We accept the following insurance plans for most of our services: Anthem Blue Cross, Aetna, Optum, United Healthcare, and Medicare. Please contact us for information on coverage for the services you seek. Our intake team can help you figure out the details of your plan and coverage, or refer you to our self-pay cash rates if you are out-of-network or seek services for which we do not accept insurance. We help verify benefits as a courtesy, but benefit information is not guaranteed until confirmed by your insurance company. Please note that insurance companies typically do not cover treatment options that are considered to be experimental, such as brain training services.

Payments
You may bring your payment to each session to give directly to your provider, or you can pay online prior to your scheduled session. We accept cash, checks made out to “Crossroads Institute”, and all credit cards. To pay by credit card, please click Make Payment Now, or provide your details to your provider at the time of service for recurring payments. Clicking Make Payment Now will redirect you to a screen where you can enter your payment information.

Cancellation Policy
Please note that we have a strict 48-hour cancellation policy. If your appointment is not cancelled at least two days in advance, you will be charged $100 in missed session fee, including for the initial session.
